1. Form submissionRequest letter and filled appraisal forms to be submitted by the applicant with recent blood reports and current list of medicines. The documents to be provided along with include, consent letters of all children and ID proof (Aadhaar card/Driving license/Passport/OCI) of the applicant. Introducing an authorized person is a must for immediate contact and for emergencies.
